BIOMECHANICS AND REHABILITATION
FOR LOW BACK, HIPS, KNEES AND ANKLES

The underlying philosophy of treatment that we incorporate here at Dallmeyer Physical Therapy is based on biomechanical and dynamic research of Stuart McGill (http://www.backfitpro.com) and Dr. Chris Powers (website reference). However, we also combine this approach to proper movement function restoration from the teachings of Vladimir Janda, MD, DSC, who emphases movement retraining, movement patterning and overall proprioceptive facilitation. We also incorporate the philosophy and treatment processes of Mackenzie, Shirly Sahurman and Dr. Bergurs (sternal syndrome). All of these are function based and are aimed at restoring movement.

Understanding how pain developed depends on the specific structure in question. IN the case of knee pain, it may be patellofemoral dysfunction or internal derangement. In many cases, biomechanical issues are at play, especially weakness of the gluteal structures due to poor activation of the gluteal mechanisms. The glutes or hips that allow stability in the knee joint as we move. By knowing the specific structure involved and why they are involved, we can develop exercise plans that are powerful and effective. Rehabilitation is directed towards new movement patterns that need to be taught and implemented.

Our evaluations examine the movement patterns and biomechanical weaknesses that are present in patients. We accomplish this with a series of manual and active testing, as well as video analysis of walking and/or running. The movement patterns at each portion of the gait are measured and analyzed to see where the default is occurring and how we can correct it. We can measure the ground reaction force at weight bearing and the shock absorption at the hip and knees. We asses hip, knee, and ankle movement as well as trunk control during running, walking, jumping, cutting and stair walking. Only then can we really see where we need to make corrections in order to reduce pain. These corrections are achieved through strength, flexibility and movement retraining exercises. There are seven levels for restoration and all of these levels have specific parameters for performance and progression. For sports related issues we have a very specific return to sport testing protocol as well as pain free daily activities. Learning these protocols well is the key to prevention of future injuries.
